We highly encourage applicants to become familiar with the Volunteer Background Screening Information page before completing this form. It will give you details about the Town's expectations.

All applicants and each group member are required to complete the Town of Herndon Volunteer Waiver. If under the age of 18 the applicant must have a parent/guardian complete their Volunteer Waiver.

Please note that all on-site volunteers must be at least 14 years old. Group Volunteers between the ages of 14-17 must be accompanied by adult chaperone(s).
